Your First Visit To Frederick Psychology Center
We understand that you may experience some uncertainty or anxiety when you arrive for your initial appointment. Be assured that we will provide a friendly orientation to our practice and the therapy process. As a start, we ask that you provide some basic demographic information. Please download the Personal Information Form by clicking on the link below, completing the form, and bringing it to your first visit. If you forget, it's O.K. Copies of the form are available in our waiting room.

When you enter our waiting room, you will see our names listed on a panel straight ahead of you. To announce your arrival, simply flip the light switch next to the appropriate name and have a seat. You will be greeted shortly. If you have any questions, feel free to contact us before you arrive. We appreciate your assistance.
